Abstract [sv]

Bakgrund: Pandemin COVID-19 påfrestade Sveriges intensivvård under våren 2020 och krävde ökade resurser från vårdpersonalen. Dessa kritiskt sjuka patienter krävde höga doser av sederande läkemedel som ledde till att patienterna hade lång vårdtid med ventilatorbehandling. Detta påverkade patienterna gällande minnen från intensivvården i form av hallucinationer och mardrömmar. Syfte: Att undersöka vilka minnen och upplevelser av hallucinationer och mardrömmar som patienter med COVID-19 haft i samband med intensivvårdstiden. Metod: Studien var en tvärsnittsstudie som genomfördes med mixad design i form av en validerad enkät: ICU Memory Tool med kvalitativa och kvantitativa frågor. 32 patienter svarade på enkäten och det utmynnade i fem kategorier. Resultat: Patienternas minnen beskrevs av fem kategorier: angripen och hindrad, syn- och ljudupplevelser, andnöd och dödsångest, påminda om COVID-19 och samtal med närstående. Slutsats: Hallucinationer och mardrömmar är ett problem inom intensivvården som leder till förlängd vårdtid, ökat lidande samt ökad mortalitet. Majoriteten av patienterna uppgav illusoriska minnen som upplevdes som ångestfyllt både under och efter intensivvårdstiden. Specialistsjuksköterskan behöver bli mer observant på tidiga tecken till delirium för att förhindra lidande för patienten genom förebyggande åtgärder i ett tidigt skede.

Abstract [en]

Background: The pandemic COVID-19 strained Sweden's intensive care in the spring of 2020 and required increased resources from the care staff. These critically ill patients required high doses of sedative drugs, resulting in long-care periods with ventilator treatment. This affected the patients regarding memories from intensive care in the form of hallucinations and nightmares. Purpose: To investigate what memories and experiences of hallucinations and nightmares that patients with COVID-19 had in connection due to intensive care. Method: This research was a cross-sectional study who conducted with a mixed design in the form of a validated questionnaire: ICU Memory Tool with qualitative and quantitative questions. 32 patients responded to the questionnaire and it resulted in five categories. Results: The patients' memories were described by five categories: attacked and obstructed, visual and audio experiences, shortness of breath and death anxiety, reminding of COVID-19 and conversations with relatives. Conclusion: Hallucinations and nightmares are a problem in intensive care that leads to extended care time, increased suffering and increased mortality. The majority of patients reported illusory memories that were perceived as anxious both during and after the intensive care unit. The specialist nurse needs to become more observant of early signs of delirium to prevent suffering for the patient through preventive measures at an early stage.
