Abstract [sv]

Neurologiskt status kan indikera om en patients neurologiska tillstånd har förbättrats, försämrats eller är oförändrat. I litteraturen saknas konsensus gällande när neurologiskt status bör utföras och i praktiken är det ofta sjuksköterskans individuella bedömning som styr beslutet. Syftet var att beskriva hur den erfarna intensivvårdssjuksköterskan grundar beslutet att utföra neurologiskt status. En fokusgruppsintervju utfördes med fem intensivvårdssjuksköterskor med mångårig erfarenhet av neurokirurgisk intensivvård. Materialet analyserades genom kvalitativ innehållsanalys och presenterades i form av tema, kategorier och underkategorier. Resultatet visade att bedömningen vid initierande av neurologiskt status grundades på intensivvårdssjuksköterskans intuition, förväntan av vad som kunde ske utifrån patientens tillstånd samt vad monitoreringen patienten var uppkopplad till visade. I bedömningen ingick vidare observation av patientens beteende samt om det fanns någon ordination på utförandet av neurologiskt status. Erfarenhet framkom som ett tema då det hade stor betydelse och gick som en röd tråd genom alla delar i bedömningen.

Abstract [en]

Neurological status may indicate if a patient's state has improved, worsened or remains unchanged. There is no consensus in literature regarding when to perform neurological status; in practice the decision is commonly made as an individual judgmentby the nurse. The purpose was to describe upon what the experienced intensive care nurse bases the decision to perform neurological status. The study was carried out with focus group interviews consisting of five intensive care nurses, each possessing several years of experience in neurological intensive care unit. The material was analyzed through qualitative content analysis, and was presented in theme, categories and sub categories. The result shows that the assessment at initiation of neurological status was based upon the intensive care nurse’s intuition, expectation of events based on the state ofthe patient, and what information patient monitoring provided. Furthermore, observation was a part of the assessment of patient behavior, as well as provided ordination regarding the performance of neurological status. Experience is the observable theme for the study, as its importance is apparent in all parts of the assessment.
