Abstract [sv]

Bakgrund: Rättspsykiatrisk vård riktar sig till personer som begått brott under påverkan av allvarlig psykisk störning. Vården bedrivs ofta i miljöer med hög säkerhet och begränsad autonomi, vilket skapar komplexa vårdsituationer för sjuksköterskor. De förväntas ge personcentrerad vård inom ramarna för juridik, säkerhet och etiska utmaningar.

Syfte: Att belysa sjuksköterskors erfarenheter av att vårda patienter inom rättspsykiatrisk vård.

Metod: En litteraturöversikt med induktiv ansats baserad på kvalitativa studier. Totalt inkluderades 13 artiklar publicerade mellan 2012 och 2023.

Resultat: Visar på att sjuksköterskor inom rättspsykiatrisk vård ofta befinner sig i en komplex och emotionellt krävande kontext. De uttrycker en ständig balansgång mellan att upprätthålla säkerhet och att erbjuda en vård präglad av empati och respekt. Flera upplever etiska dilemman, känslor av otillräcklighet samt en brist på stöd från organisationen. Samtidigt beskrivs också en stark vilja att göra skillnad för patienterna och skapa meningsfulla relationer trots de restriktioner som råder.

Slutsats: För att stärka sjuksköterskors möjligheter att ge god vård krävs ökad förståelse, organisatoriskt stöd och utrymme för reflektion. 

Abstract [en]

Background: Forensic psychiatric care is provided to individuals who have commited a crime under the influence of a severe mental disorder. This type of care is often carried out in high- security environments with limited patient autonomy, creating complex care situations for nurses. They are expected to deliver person centered care within legal, safety related, and ethical boundaries.

Aim: To explore nurses ‘experiences of caring for patients within forensic psychiatric care.

Method: A literature review with an inductive approach based on qualitative studies. A total of 13 articles published between 2012 and 2023 were included.

Results: The findings show that nurses in forensic psychiatric care often work in complex and emotionally demanding contexts. They describe a constant balancing act between maintaining safety and providing care characterized by empathy and respect. Many report ethical dilemmas, feelings of inadequacy, and lack of organizational support. At the same time, there is a strong desire to make a difference for patients and to build meaningful relationships despite existing restrictions.

Conclusion: To support nurses in delivering high quality care, increased understanding of their role, improved organizational support, and opportunities for reflection are needed.
