Abstract [sv]

Bakgrund: Omvårdnad i livets slutskede är en vanlig uppgift på en intensivvårdsavdelning. Sjuksköterskor behöver anpassa sig från en livräddande insats till att vårda patienten i livets slutskede. De ställs då inför en komplex uppgift med att värna om patientens värdighet och möjliggöra en fridfull död i en tekniskt avancerad vårdmiljö.

Syfte: Att beskriva intensivvårdssjuksköterskors erfarenheter av omvårdnad av vuxna patienter i livets slutskede på intensivvårdsavdelning.

Metod: Studien utfördes som en allmän litteraturstudie med en kvalitativ design baserad på nio vetenskapliga originalartiklar. Dataanalysen utgick ifrån en reflexiv tematisk analys enligt Braun och Clarke.

Resultat: Två teman identifierades: upplevda utmaningar för en värdig och personcentrerad omvårdnad samt emotionella upplevelser och utmaningar. Dessa omfattade fyra subteman: vårdmiljön som en utmaning i omvårdnaden, utmaningar i kommunikationen och i den andliga omvårdnaden, upplevda emotioner vid omvårdnaden i livets slutskede samt upplevelsen av etisk stress.

Slutsatser: Intensivvårdssjuksköterskor upplevde omvårdnaden i livets slutskede som både emotionellt belastande och meningsfull. Hinder identifierades i form av vårdmiljö, kommunikativa svårigheter och kulturella skillnader. Resultaten indikerar ett behov av fortsatt forskning, utbildning och organisatoriska förbättringar för att stödja omvårdnaden i livets slutskede på intensivvårdsavdelningar.

Abstract [en]

Background: End-of-life care is a common responsibility in intensive care units. Intensive care nurses must navigate the transition from life-saving interventions to providing care for patients at the end of life. They face the complex task of preserving patients’ dignity and facilitating a peaceful death within a technologically advanced care environment.

Aim: To describe intensive care nurses’ experiences of caring for adult patients at the end of life in the intensive care unit.

Method: The study was conducted as a general literature review with a qualitative design, based on nine scientific original articles. Data analysis was conducted using a reflexive thematic analysis according to Braun and Clarke.

Results: Two themes emerged: perceived challenges in providing dignified and person-centred care and emotional experiences and challenges. Four subthemes were identified: the care environment as a challenge in nursing care, challenges in communication and in spiritual care, experienced emotions in end-of-life care, and the experience of ethical stress. 

Conclusion: Intensive care nurses experienced end-of-life care as both emotionally demanding and meaningful. Barriers were identified related to the care environment, communication difficulties, and cultural differences. The findings indicate a need for further research, education and organizational improvements to support end-of-life care in intensive care units.
