Abstract [sv]

Bakgrund: Palliativ vård i livets slutskede syftar till att lindra lidande och erbjuda en god livskvalitet under sista en i livet. Behovet av palliativ vård ökar och många patienters önskan att avsluta livet i hemmet. Vård i livets slutskede i hemmet ställer särskilda krav på sjuksköterskor att tidigt kunna identifiera patientens behov, kommunikation, samordning samt ge stöd till både patient och närstående.  

Syfte: Syftet med studien var att beskriva sjuksköterskans erfarenheter av omvårdnad av patienter i livets slutskede i hemmet.   

Metod: Denna studie utfördes som en allmän litteraturstudie med kvalitativ ansats. 10 artiklar hämtades från databaserna CINAHL och Pubmed. Studien följer Polit & Becks 9 stegs modell. Analysen genomfördes med hjälp av Braun & Clarks 6 stegs modell.    

Resultat: Två huvudteman identifierades: Etiska och relationella utmaningar i hemsjukvården, professionella och organisatoriska förutsättningar för palliativ vård i hemmet. Två subteman per tema identifierades: etiska dilemman i omvårdnaden, familjens påverkan på vårdandet, arbetsbelastning och begränsade resurser, kompetens och behov av stöd.  

Slutsats: Att arbeta med vård i livets slutskede i hemmet som sjuksköterska involverar komplexa etiska, relationella och organisatoriska utmaningar. Sjuksköterskors erfarenheter belyser behovet av att hantera de etiska utmaningar och organisationens förutsättningar samt resurser för att kunna erbjuda en högkvalitativ vård i livets slutskede i hemmet.    

Abstract [en]

Background: Palliative care at the end of life aims to relieve suffering and promote a good quality of life during the final stages of life. The need for palliative care is increasing, and many patients wish to spend their final days at home. Providing end of life care at home places specific demands on nurses, including the ability to identify patient’s early needs, ensure effective communication and coordination and provide support to both patients and their families.   

Aim: This study aimed to describe nurses experiences of caring for patients at end of life in their homes.   

Method: This study is a literature review with a qualitative approach. Articles were retrieved from CINAHL and Pubmed databases following Polit and Beck´s nine-step model. Ten scientific articles were selected and analyzed using a qualitative content analysis inspired by Braun and Clares´s thematic analysis to identify recurring themes.  

Results: The results revealed two main themes: Ethical and relational challenges in home healthcare, professional and organizational conditions for palliative care at home, with two subthemes each: ethical dilemmas in nursing, family´s influence on care, workload and limited resources and competence and need for support.

Conclusion: Providing end-of-life care at home as a nurse involves complex ethical, relational, and organizational challenges. Nurses experience highlight the importance of managing ethical dilemmas and demonstrate how organizational conditions and access to resources can contribute to high quality end-of-life care in the home setting.
