Abstract [sv]

Bakgrund: Bipolär sjukdom är en kronisk psykiatrisk sjukdom som kännetecknas av återkommande episoder av mani, hypomani och depression samt perioder av stabilt stämningsläge. Sjukdomen kan påverka individens funktionsförmåga och livskvalitet samt medföra svårigheter att leva ett stabilt vardagsliv. Bipolär sjukdom innebär ofta komplexa vårdbehov och kan påverka flera aspekter av patientens liv såsom sociala relationer, arbetsförmåga och psykiskt välbefinnande. Personer med bipolär sjukdom är ofta i behov av återkommande och långvarig psykiatrisk vård i både öppenvård och slutenvård, där även tvångsvård kan bli aktuellt. Behandlingens mål är att stabilisera stämningsläget, lindra symtom och förebygga återfall. Syfte: Syftet var att beskriva patienters upplevelser av omvårdnadsåtgärder inom öppen och sluten psykiatrisk vård vid bipolär sjukdom. Metod: Uppsatsen är en strukturerad litteraturstudie med kvalitativ ansats. Resultat: Studien visar att patienter med bipolär sjukdom har olika upplevelser av omvårdnadsåtgärder beroende på vilket tillstånd de är i. När patienter är maniska uppskattar de struktur extra mycket medan när de är deprimerade behöver de snarare emotionellt stöd. Patienterna uppskattade en bra vårdrelation och den är avgörande för att patienter med bipolär sjukdom ska känna trygghet och delaktighet i sin vård. En bra vårdrelation utifrån patientens behov utgörs av tydlig kommunikation, patientcentrerad vård och struktur. Omvårdnadsbehovet är skiftande och varierar beroende på sjukdomens skede. Individanpassad information och delaktighet i behandlingen framträder som centrala faktorer som kan främja följsamhet till behandling.   Slutsats: En tillitsfull vårdrelation, delaktighet och individanpassad omvårdnad är centrala faktorer för en positiv vårdupplevelse. Detta kan främja följsamhet till behandling, medan brister i dessa faktorer samt stigma kan hindra återhämtning hos patienter med bipolär sjukdom.   

Abstract [en]

 ABSTRACTBackground: Bipolar disorder is a chronic psychiatric illness that involves repeated episodes of hypomania, mania, depression but also stable moods so called euthymic periods. The disorder can affect patients’ quality of life and overall functionting. These patients need regular psychiatric care, both as outpatients and inpatients, which can be voluntary and/or involuntary care. The care can be complex due to the varying nature of the illness. The goal is to help patients reach euthymic periods, but with this illness it can be difficult for patients to have stability in their lives.  Aim: The aim of the study in bipolar disorder was to describe patients’ experiences of nursing interventions in both outpatient and inpatient psychiatric care. Method: The essay is a structured literature study with a qualitative approach. Main results: The study showed that patients with bipolar disorder experienced nursing care in different ways depending on what phase in the illness they were in. During a depressive episode patients appreciated emotional support but during a manic episode they experienced that structure was one of the most important needs. Patients had a positive experience with nursing care when trust, clear information, and individualized care were in focus. Barriers such as stigma, limited involvement, and poor communication affected patients' autonomy negatively and led to more difficult recovery and treatment adherence. Conclusion: Patients' experience of mental health nursing care in bipolar disorder varies due to fluctuating mood states, voluntary or involuntary care, cognitive function, social support and socioeconomic factors. The nurse's competence and experience affected the quality of the care and patients' experience of nursing care. In summary, individualized care, person-centered care and nurses that can see the patient beyond the diagnosis was crucial for patients to have a good experience of nursing care.
